The AfyaPro intelligent hospital system and the AfyaPro mobile App and e-Care Centre, operate on one and the same platform and exist to manage Cardio-diabetes-hypertension conditions. 

How the App works 

To provide tele coaching and tele care through an e-care centre. The e-Care centre monitors the patient condition based on an electronic medical record (EHR). The patient is monitored, coached and advised by a nurse (or a medical student) and supervised by a medical doctor.  

The patient is connected through an App. Either the patient, family, nurse, pharmacy measures regularly key values (blood sugar etc.) as needed and registers in the App which uploads to the EHR. The Patient can answer questionnaires and do educational tasks like what is diabetes, what is the diet etc. 

All care takers have instant access to the EHR, this implies Doctors can work from different locations.
